  • Advances in the study of prognostic factors for papillary thyroid carcinoma

    ObjectiveTo explore the factors that predict the prognosis of papillary thyroid carcinoma.MethodThe literature searching terms include "prognosis of papillary thyroid carcinoma" "ultrasound of papillary thyroid carcinoma" "papillary thyroid carcinoma and Hashimoto's thyroiditis", and "genes related to papillary thyroid carcinoma". The factors used to predict the prognosis of papillary thyroid carcinoma such as the patient's clinical characteristics, ultrasound characteristics, serological indicators, genes mutations, etc. were reviewed.ResultsUltrasound features such as lesion diameter greater than 1 cm, extra-glandular invasion, and related gene mutations may increase the risk of postoperative recurrence. Patients with Hashimoto's thyroiditis may have better prognosis.ConclusionThere are many factors that can predict the risk of recurrence in patients to a certain extent, but most of them are just in the study, so it is necessary to enact more standardized follow-up management to improve the survival rate.

  • Effect of Hepatitis B Immunoglobulin Intrauterine Injection on Interrupting Hepatitis B Virus Mother-to-Child Transmission: A Systematic Review

    Objectives To evaluate the effects of hepatitis B immunoglobulin (HBIG) intrauterine injection before delivery on interrupting mother-to-child transmission of hepatitis B virus (HBV). Methods The randomized controlled trials (RCTs) on HBIG intrauterine injection on interrupting mother-to-child transmission of HBV published between January 1992 and May 2012 were searched in The Cochrane Library, PubMed, CBM, CNKI, VIP, WanFang Data, etc. The studies were screened according to inclusive and exclusive criteria, the data were extracted, the quality was assessed by two reviewers independently, and meta-analysis, publication bias and sensitivity analysis was conducted using Stata software. Results The total 42 studies involving 7 212 infants were included. The randomized methods were asserted in all studies, three of which reported the details of randomization, one study mentioned blinded method, two studies mentioned incomplete outcome data, 13 studies had other potential threats to validity, no allocation concealment and selective outcome reporting was mentioned. Results of meta-analysis indicated that the infant HBV infection rates in the HBIG group and the control group were 8.971% and 25.470% (RR=0.359, 95%CI 0.303 to 0.425) at birth, 5.385% and 13.919% (RR=0.391, 95%CI 0.278 to 0.550) after half a year, 5.318% and 12.457% (RR=0.429, 95%CI 0.335 to 0.551) after one year; the infant anti-HBs rates in the HBIG group and the control group were 61.964% and 14.523% (RR=6.712, 95%CI 1.920 to 23.467) at birth, 77.754% and 66.311% (RR=1.209, 95%CI 0.989 to 1.478) after half a year. Funnel graphs showed that there was publication bias. Sensitivity analysis showed that the results except the infant anti-HBs protection after half-a-year follow-up were stable and consistent with the original results. Conclusion Injection of HBIG during pregnancy for HBV-carrying mothers can effectively reduce the occurrence of HBV whenever at birth, after half a year or after one year, and increase the infant anti-HBs protection rate at birth, but it is ineffective to improve anti-HBV protection rate after half a year. Owing to the low quality of the included studies and existence of biases, this conclusion should be cautiously put into clinical practice.

  • Effect of RNA Interference for c-Jun Gene on Proliferation of Rat Vascular Smooth Muscle Cells

    Objective To investigate the influence of RNA interference targeting c-Jun gene on the proliferation of rat vascular smooth muscle cells (VSMCs). Methods The experiment was performed with c-Jun siRNA (c-Jun siRNA group), control reverse sequence siRNA (control siRNA group) or no siRNA (control group). VSMCs were transfected with siRNA targeting c-Jun gene by liposome. Effects of c-Jun siRNA on mRNA and protein expressions of c-Jun were examined by RT-PCR analysis and Western blot respectively. MTT test and 3H-TdR incorporation were used to detect VSMCs proliferation. Cell cycle analysis of VSMCs in vitro was determined by flow cytometer. Results The expression levels of mRNA and protein of c-Jun in c-Jun siRNA group were significantly lower than those in control group (P<0.05, P<0.01). There was no significant difference between control group and control siRNA group (Pgt;0.05). Proliferation activity of VSMCs decreased significantly in c-Jun siRNA group compared with that in control group (P<0.05) and VSMCs was blocked in the G0/G1 phase of cell cycle significantly (P<0.05). There was no significant difference between control group and control siRNA group (Pgt;0.05). Conclusion c-Jun gene silenced by RNA interference can inhibit VSMCs proliferation effectively in vitro.

  • Research progress of immune response in infection associated with severe acute pancreatitis

    ObjectiveTo summarize progress of immune response in severe acute pancreatitis (SAP) and to provide a basis for appropriate immunotheraphy.MethodThe relevant literatures about the effect of immune response in the SAP with infectious complications in recent years were reviewed.ResultsThe inflammatory cascade reaction occurred in the early stage of SAP. Subsequently, the compensatory anti-inflammatory response syndrome (CARS) arised and immune response of the organism was suppressed. At this stage, the rate of infection was higher than before.ConclusionsCARS is one of major reasons in SAP with infectious complications. At present, fluid infusion, fasting, parenteral nutrition and like are major therapies in SAP. If corresponding immunotherapy could be carried out according to immune mechanism of SAP infection, that is, early appropriate immunosuppressive therapy and dynamic monitoring of body’s immune system state should be performed, when it is found that immunosuppression is present, appropriate immunostimulus therapy will be possible to reduce mortality of SAP and improve its prognosis.

  • The role of alternative splicing in autoimmune diseases

    Alternative splicing plays an important role in the pathogenesis, diagnosis, treatment and prognosis of autoimmune diseases. Alternative splicing is universal and non-preferred in autoimmune diseases, and exon skipping is the most common type in alternative splicing types. The occurrence and development of autoimmune diseases can be influenced by the 5′ splicing, 3′ splicing, number change of exons, splicing affected by the single nucleotide polymorphism and the variance of gene expression levels. Moreover, different single nucleotide polymorphisms of the same gene can affect the development of various autoimmune diseases. This review summarizes the role of different forms of alternative splicing in various autoimmune diseases, and aims to provide a basis for further study of the conditions in different development stages of autoimmune diseases and the regulatory mechanism of different levels of splicing isoforms.

  • Research on self-management behavior and influencing factors among maintenance hemodialysis patients

    ObjectiveTo investigate the level and influencing factors of self-management behaviors among maintenance hemodialysis patients.MethodsPatients undergoing maintenance hemodialysis were recruited from 4 tertiary hospitals in Chengdu from October 2016 to October 2017. Convenient sampling method was used to select patients. The instruments used in the study included General Information Questionnaire, Hemodialysis Self-management Instrument, Hemodialysis-related Knowledge Questionnaire, Herth Hope Index, Multidimensional Health Locus of Control Scale, and Chronic Illness Resources Survey.ResultsA total of 360 patients were investigated, and 343 patients were finally included. The mean score of self-management behavior was 54.87±9.65. The results of multiple linear stepwise regression showed that gender, age, ethnic, monthly family income, chronic illness resources, knowledge and internal health locus of control were the significant influencing factors of self-management behaviors (P<0.05).ConclusionThe self-management behavior among patients undergoing maintenance hemodialysis can be promoted by improving the level of knowledge and internal health locus of control, and making effective use of chronic illness resources.

  • oXiris - Clinical practice of endotoxin adsorption

    Sepsis is life-threatening organ dysfunction caused by a dysregulated host response to infection, and is the leading cause of death in intensive care unit patients due to limited therapies currently. As is known to all, endotoxin is important in the pathogenesis of sepsis, whereby infection triggers a systemic inflammatory response, resulting in release of pro- and anti-inflammatory cytokines, which are thought to be responsible for the tissue damage that occurs in sepsis patients. Therefore, removing endotoxins is considered as an effective way to improve the conditions of sepsis patients. Both Toraymaxin (PMX) and adsorptive membrane such as oXiris can remove endotoxins by adsorption, and in this review we will summarize current studies, included in vitro study, animal study, and clinical research to show the benefit from endotoxin removal by oXiris, and also give some suggestions about oXiris clinical practice from experienced experts.

  • The value of platelet volume indices in early period as prognostic indicators of sepsis

    ObjectiveTo investigate the prognostic value of platelet volume indices [mean platelet volume (MPV) and platelet distribution width (PDW)] in early period for patients with sepsis .MethodsThis retrospective study was conducted in a general ICU at a University Hospital from January 2015 to January 2017. Patients with sepsis were divided into a survival group and a death group according to 28-day prognosis. The demographic and clinical data were recorded and analyzed, including platelet indices, Charlson comorbidity index, Glasgow Coma Scale (GCS) score and Acute Physiology and Chronic Health Evaluation Ⅱ (APACHEⅡ) score. Multivariable COX proportional hazard ratio model was used to evaluate the 28-day mortality risk, and receiver operating characteristic (ROC) curve were used to evaluate the value of platelet volume indices.ResultsA total of 209 patients were enrolled in this study. Mean arterial pressure, pH, platelet count, serum creatinine, bilirubin, troponin I, prothrombin time, GCS score and platelet volume indices showed significant differences between the survival group (n=137) and the death group (n=72) (all P<0.05). Multivariable COX regression analysis revealed that the PDW was independently associated with 28-day mortality in sepsis (HR=1.068, 95%CI 1.002-1.139, P=0.043). The area under the ROC curve of PDW was 0.762 (95%CI 0.694 - 0.831), with a sensitivity of 86% and a specificity of 63% at a cut-off value of 18.65%.ConclusionPDW can be used as a powerful prognostic indicator in patients with sepsis following 28-day.

  • Epidemiology Analysis of the Distribution of People with Trauma due to Traffic Accidents in Deyang City

    目的 探讨德阳市道路交通伤中人员分布的流行病学特点。 方法 回顾性统计分析德阳市2003年-2005年发生的5 300例道路交通伤资料,总结其中的规律和特点。 结果 在5 300例交通伤中,伤亡人员以男性居多(男︰女= 2.58︰1),其中16~55岁的青壮年占了全部伤亡人员的75.72%;工人、农林牧渔业人员和学生占交通伤的比例最高,达52.73%。步行、二轮摩托车和自行车是造成人员伤亡的最主要三个原因,三者比例达到了59.34%。 结论 加强交通安全意识的教育,加大交通法规的宣传和贯彻力度,加强部门间协作是减少交通伤发生的有效措施。Objective To investigate the epidemiological characteristics of the distrubution of people with trauma due to traffic accidents in Deyang City. Methods The data of 5 300 road traffic accidents from 2003 to 2005 in Deyang were retrospectively analyzed. The rules and characters were summarized. Results More males were injured or dead than females in the 5 300 road traffic accidents (male : female = 2.58︰1 ). The young adults aged from 16 to 55 accounted for 75.72% of all the casualties. The workers, agriculture employees and students had the largest percentage (52.73%) of the injuries. Walking and riding motorcycles and bicycles were the main causes leading to the injury, which occupied 59.34%. Conclusion The effective measures to reduce trauma due to traffic accidents will attribute to enhance the personnel awareness of road safety education, improve the publicity work and enforcement of traffic laws, and strengthen collaboration of different epartments.

  • Comparison of quality of life in patients with advanced Siewert type Ⅱ adenocarcinoma of the esophagogastric junction treated by three different operations

    Objective To compare the quality of life (QOL) of advanced Siewert type Ⅱ adenocarcinoma esophagogastric junction (AEG) patients treated by the total gastrectomy, traditional proximal gastrectomy or proximal gastrectomy with gastric tube reconstruction opertations, and to provide some clinical basis for the choice of surgical methods for AEG. Methods A total of 90 patients with Siewert type Ⅱ AEG were retrospectively collected from the Affiliated Hospital of North Sichuan Medical College. Patients were divided into 3 groups according to different surgical methods (n=30 in each group), a total gastrectomy group (23 males, 7 females, aged 47-79 years), a traditional proximal gastrectomy group (treated with the traditional proximal gastrectomy procedure, 19 males, 11 females, aged 44-80 years), and a narrow gastric tube group (treated with the proximal gastrectomy with gastric tube reconstruction procedure, 25 males, 5 females, aged 47-83 years). The Chinese version of Quality of Life Questionnaire Core-30 (QLQ-C30) and Quality of Life Questionnaire Oesophagogastric-25 (QLQ-OG25) designed by European Organization for Research and Treatment of Cancer (EORTC) were used to collect the patients’ information in the three groups about their QOL during the first six months and one year after the three procedures. Results There was no statistical difference in the clinical data among the three groups (all P>0.05). QOL during the first six months after the operations assessed by the QLQ-C30 questionnaire table showed that the narrow gastric tube group was significantly best in total QOL, physical function, fatigue, and emotional function among the three groups (all P<0.05). The total gastrectomy group was the worst in role function, dyspnea, fatigue and diarrhea among the three groups (all P<0.05). The traditional proximal gastrectomy group had a worse evaluation in lose of appetite than the other two groups (P<0.05). QOL during the first six months after the operations assessed by the QLQ-OG25 questionnaire table showed that the traditional proximal gastrectomy group had a significantly worse evaluation in palirrhea than the other two groups (both P<0.05). The total gastrectomy group had a significantly worse evaluation in anxiety than the other two groups (both P<0.05). QOL during the first year after the operations assessed by the QLQ-C30 questionnaire table showed that the narrow gastric tube group had a significantly highest evaluation in total QOL physical function and emotional function among the three groups (all P<0.05). The total gastrectomy group had a significantly worst evaluation in diarrhea among the three groups (P<0.05). QOL during the first year after operations assessed by QLQ-OG25 questionnaire table showed that the traditional proximal gastrectomy group had a significantly worse evaluation in palirrhea than the other two groups (all P<0.05). Conclusion The narrow tubular esophagogastric anastomosis is better than the total gastrectomy and the traditional proximal gastrectomy for the treatment of the advanced Siewert type Ⅱ adenocarcinoma of esophagogastric junction, so this operation is worth being recommended.
